Funeral Planning Documents
Planning for a funeral is not something we look forward to with joy and anticipation. However, as Christians, we know that death awaits us—not as a tragic ending—but as a passage into a new and indescribably marvelous life with God. As St. Paul says:
“What eye has not seen, and ear has not heard, and what has not entered the human heart, what God has prepared for those who love him” (1 Cor 2:9-10).

Marriage Preparation Guide and Policies
Congratulations on your upcoming marriage! Marriage is a serious and sacred commitment. Through it, husband and wife encounter God who draws husband and wife closer to each other in a life-giving community of love.
Our Parish, along with the Toledo Diocese, has established a program of marriage preparation to support couples in the adventure of married life. The following pages contain the policies for Saint John the Baptist Catholic Parish for weddings and marriage preparation. It is our hope and prayer that your marriage preparation and wedding will lead you to deeper knowledge of God’s love for you!
Baptismal Certificate Form for Infants
Baptism is the first sacrament of initiation into the Catholic faith. Through the waters of Baptism, we are cleansed of original sin, become members of the Body of Christ, and are welcomed into the Church family.
Since Baptism is the first of the Sacraments, with it begins a Christian’s sacramental record in the Church. The Baptismal Certificate Form for Infants must be filled out and submitted at least 10 days before the date of your child’s Baptism.
